By submitting this application, you understand this is the first step of the adoption process. Fetching Hope Rescue reserves the right to refuse adoption to anyone. You certify the information contained within this application is accurate and not misleading in any way. Our organization reserves the right to contact individuals listed on this form for verification.
By signing this application, you understand that we make every effort to ensure every animal is healthy and all records will be passed forward to the adopter, but FHR is not responsible for future vet care. We make no guarantees in regards to health or temperament and our goal is to provide the most knowledge possible about each dog, but you will be adopting your dog “as is.” Due to the fact that most of our adoptable animals are rescued as strays, we cannot guarantee their breed. We list the pets breed/s as the one/s that best matches their appearance and personality.
Every dog arrives spayed/neutered, microchipped, with medical records, along with the interstate health certificate to accompany the animal on transport. Every dog is current on age-appropriate vaccinations: DA2PPv Canine Distemper, Bordetella, Rabies (if over 3 months), Leptospirosis (if over 6 months) receive a Heartworm test (if over 6 months). Every dog must test negative for fecal worms, coccidia and giardia, as well as be given flea/tick preventative before boarding the transport truck.
By submitting this application, you understand this is the first step of the adoption process. If approved for adoption for any dog with us, by signing below, you also agree that if the adoption takes place and you are unable to keep your dog for any reason, the dog will be returned to our organization, Fetching Hope Rescue and will not be returned to a shelter or re-homed privately. We will refund adoption fees (minus a $50 processing fee) within the first two weeks, if the dog placed is not a right match for your family. If a return does occur, we ask that you are able to hold the dog until appropriate foster home is found, up to two weeks maximum. Information regarding returns and contact will be provided in official approval letter. This is a legally binding contract.
